Social determinants of health—the social, political, and economic factors that influence or define the circumstances in which people are born, grow up, live, work and age—may account for up to 80% of health determinants, according to County Health Rankings data. In order to develop effective strategies for improving population health in rural communities, it is necessary to understand the unique characteristics of those communities. In this project, the Flex Monitoring Team will identify CAHs that are actively working with engaged communities to address social determinants of health in their local service areas and highlight effective strategies those CAHs have developed in resource-constrained environments.
