• Critical Access Hospital Fact Sheet - This Fact Sheet is designed to provide additional information about the background and components of the Assessment for CAHs.
• 2024 Assessment Questions & Instructions - PDF - This document includes all of the questions on the Assessment, as well as instructions for each question. CAHs should use this to review the questions for the Assessment prior to going to the portal to complete their submission. This version can be easily shared and not easily manipulated in order to maintain all of the questions and instructions for review.
• 2024 Assessment Questions & Instructions - Word Document - This document is an alternate version of the instructions document that can be used by CAHs to fill in answers as they collect data, before they submit their responses on the Assessment portal. 
• Assessment Information Recording - This recording provides detailed information about the background, purpose, and content of the Assessment for CAHs to learn more about the Assessment prior to completing it.
• 2023 Assessment National Report - This report uses 2023 Assessment data to provide a national overview of some key components of the Assessment.
