This study will examine rural hospital transformation programs designed to improve the performance of Critical Access Hospitals (CAHs) and other rural hospitals. These transformation programs are designed to help CAHs and other rural hospitals better serve the needs of their communities by enhancing their sustainability and financial viability, improving the health of their patients, managing chronic conditions, and maintaining their ability to provide necessary healthcare services.
