<p>This is a recording of a webinar which took place on December 2, 2017. It was produced by FMT research staff at the University of Southern Maine as part of their <a href="http://www.flexmonitoring.org/projects/evaluation-state-flex-program-im… to evaluate the Flex Program's Area Five, Integration of Innovative Health Models</a>, which focuses on rural health system innovations to address needs not covered by the other core areas of the grant program.</p>
<div>This webinar focuses on care coordination and quality improvement. Speakers include: Pat Schou (Illinois); Jill Oesterle (Michigan); Bill Mize and Richard Taylor (Tennessee); and Marcy Cameron (Colorado). Schou discusses the Illinois Rural Community Care Organization's efforts to improve communication between health care facilities when patients are being transferred, as well as their efforts to improve care coordination for patients with Congestive Heart Failure and Chronic Obstructive Pulmonary Disorder. Oesterle talks about the newly formed Michigan Rural Health Clinic Quality Network. Mize and Taylor discuss efforts to help rural practices in Tennessee prepare for value-based purchasing, through targeted technical and operational assistance. Finally, Cameron talks about Colorado's iCare (Improving Communication and Readmissions) program, which looks to improve communication, reduce readmissions, and improve clinical processes.</div>
